RETURNING STUDENT | 2024-2025

A new student registration means you DO NOT have any children currently enrolled in the United Community School District.

The registration process must be completed for all students before they may attend school. A student is not considered fully registered until all of the steps listed below have been completed.

It is recommended you complete the registration process on a laptop or desktop computer, as the process can be difficult on a mobile device.

Please read through all instructions before beginning the process.

Step 1: REGISTRATION

• Complete the online registration application HERE.
• Complete all forms.
• Be sure to click "Submit" after you complete all forms.
• Provide proof of the student’s age: copy of a birth certificate, US passport, adoption papers, or custody document.
• Provide an up-to-date immunization record.
• Provide proof of physical or well-child visit (ONLY required for PRESCHOOLERS) to the school nurse.
• Provide proof of residency: utility bill, bank record, lease or mortgage document.

The required documentation can be attached to an email and sent to Karla Keigley, a can be a hard copy that is mailed to the school (1284 U Ave, Boone, Iowa, 50036), or dropped off at the school main office.

Step 2: OTHER REQUIRED REGISTRATION FORMS – Please make sure you complete these forms as well.
These forms include field trip permission, electronic device permission, medication forms, bus transportation form, etc. so it is important to complete them as well.
These forms are NOT found in Infinite Campus...click on the links below.
*Forms are REQUIRED FOR EACH STUDENT.

• Complete the Supplemental Registration Form (required). *
• Complete the Health Service Form (required).*
• Complete the Medication Form (complete ONLY if your child needs to take medication at school).
• Complete the Transportation Form (required). *
• Complete the Home Language Survey (must be printed and a hard copy turned in/sent to the school) (required). *
• Certificate of Vision Screening (ONLY required for Developmental Kindergarteners, Kindergartners and 3rd grade students).
• Certificate of Dental Screening (ONLY required for Developmental Kindergarteners and Kindergartners). 
• Complete the Confidential Information Form (required). *
• Free and Reduced School Meal information (not required but encouraged).

Step 3: SET-UP A USER ACCOUNT IN INFINITE CAMPUS

• Once you have completed Step 1, you will receive an email from Infinite Campus. That email will contain a link to the United Parent Portal set-up page.
• Follow the instructions to create your Parent Portal account.

Step 4: FEES

No fees are due at this time. Fees for the 2024-2025 school year will be posted in your Infinite Campus Parent Portal sometime after October 1, 2024. You will be notified when the fees are posted in your portal.
Free and Reduced Meal applications will be available during July for the 2024-2025 school year. If you are applying for meal assistance, please wait to pay your registration fees until you have been notified of your meal assistance approval or denial, as this can affect your registration fees.

• Log in to your Infinite Campus Parent Portal.
• If you have questions about your Parent Portal account, please contact Karla Keigley or Ann Howe-Gorden.
• Select “More” from the left navigation menu.
• Click on "Online Registration"
• Choose "Existing Student"
• Complete all forms.
• Add any new additional students to be enrolled for the 2024-2025 school year under the “Student” section (example: Preschoolers or Kindergartners).
• Be sure to click "Submit" after you complete each form.
